On day 13 of mechanical ventilation, a thoracic CT scan showed diffuse bilateral
alveolar infiltrates with dilatation and distortion of bronchial structures.
A chest CT showed findings of pulmonary thromboembolism,
alveolar infiltrates in the left lung and interstitial fibrotic lesions, bulla, bilateral pleural effusion and right cardiomegaly (Figure 1 C).
Chest radiograph done in our hospital showed diffuse bilateral
alveolar infiltrates in the upper lobes, perihilar region, and in the lower lobes (Figure 1).
PcP was suspected in patients with clinically advanced HIV presenting with hypoxic respiratory failure with typical chest radiograph changes, including bilateral diffuse
alveolar infiltrates, granular opacities or, occasionally, unilateral or focal infiltrates.

Physical exam may reveal wheezing or crackles and chest radiograph can show interstitial or
alveolar infiltrates, as well as pleural effusions.
C develops hemoptysis, and chest radiography shows increasing

Lung injury includes acute bronchitis(10,13),
alveolar infiltrates, cavitating lesions generally less than 2 cm in diameter (30%) and pleural effusions (25%)(14).
(11) When a patient does seek medical attention, the chest x-ray might show a variety of pulmonary parenchymal lesions, including local consolidation (28% of patients), mass-like lesions (23%), diffuse
alveolar infiltrates (13%), and cavitary lung lesions (10%).
Radiological examination of the lungs often reveals a characteristic batwing appearance resulting from bilateral
alveolar infiltrates. Symptoms of PAP closely resemble other pulmonary diseases, including asthma, pulmonary edema, and infection with Pneumocystis carinii (Claypool, Rogers, & Matuschak, 1984).

Chest radiographs within 24 hours of admission indicated bilateral
