After the osteotomy, the cage method was performed with a polyetheretherketone cage [Figure 2] (Weigao Orthopedic, Shandong, China) and bone autograft packed into the osteotomy space at the anterior column
of the vertebral body.
The ALIF ACR procedure is an alternative method of treating sagittal imbalance from the anterior column
, and may provide the same correction capability with decreased complications and morbidity.
A relative reduction may occur with either: 1) large tongue volume relative to the volume of the bony limits of the anterior column
or 2) prominent top incisors requiring more mandibular protrusion during direct laryngoscopy.
Because the anterior columns
remain intact, this fracture is considered mechanically stable.
For our patient, the Pfannenstiel approach was used to stabilize the anterior column
of the acetabulum fracture .
All fracture types based on Letournel and Judet classification involving the anterior column
, anterior wall, posterior column, and posterior wall, except the ones mentioned in the exclusion criteria, were included.

By utilizing an oblique lateral approach to the spine, this procedure enables placement of a large interbody graft into the disc space for anterior column

For the long-term health of soil tissues and facets, the elasticity of the anterior column
unit should be restored to normal physiologic ranges.

In the anatomical three-column model described by Denis (5), the anterior column
includes the anterior longitudinal ligament and the anterior half of the vertebral body and the annulus fibrosus, the middle column includes the posterior half of the vertebral body and the annulus fibrosus along with the posterior longitudinal ligament, and the posterior column includes the bone and ligamentous structures posterior to the posterior longitudinal ligament.
Although instrumented posterior fusions provides motion segment stability in addition to neural decompression, it does not reconstruct the anterior column
and may even exacerbate kyphosis.
